Service Description

Recording & Reporting is a practical, interactive course exploring one of the most important, and often underestimated, responsibilities in residential childcare: creating records that accurately reflect a child's experiences, the actions of staff and the decisions being made about their care. Residential childcare generates a significant amount of written information. Daily logs, incident reports, safeguarding concerns, key-work records, handovers and care documentation all contribute to the professional understanding of a child's life. Good recording is therefore much more than completing paperwork. Participants explore how to create records that are clear, accurate, objective, meaningful and useful. The course examines the difference between fact, observation, interpretation and opinion, demonstrating how seemingly small choices in language can significantly change the meaning of a record. Practitioners consider the impact of vague, judgemental or institutional language and explore how records can remain professional while still capturing the child's voice, experience and individuality. The course also examines the safeguarding importance of recording. A single observation may appear insignificant in isolation, but accurate records can reveal patterns, changes and cumulative concerns when information is considered over time. Participants explore what should be recorded, what detail matters, how to describe behaviour objectively and how to evidence actions, decisions and professional reasoning clearly. Realistic examples and practical exercises allow participants to examine weaker records, identify problems and consider how they could be strengthened. Ultimately, the course encourages practitioners to see recording differently. A good record does not simply prove that something was written down. It helps others understand what happened, why it mattered, what action was taken and what may need to happen next. Following the session, each participant completes an individual Learning Validation & Reflection. Every submission is assessed before certification, with a complimentary 30-minute 1:1 follow-up available where further learning is required.
